Your final paycheck will include payment for all time worked through your last day of employment.
Final salary payments are issued in accordance with applicable state requirements and no later than the next scheduled pay date following your termination date.
Sales incentive plans
If you are enrolled in a sales incentive plan, your final incentive payment will be calculated according to the applicable Sales Incentive Plan terms and conditions. Access to Callidus will end on your separation date.

ADP Workforce access
Hourly employees are not required to record hours in ADP during the salary continuation period.
You can access your Pay Statements and W-2 tax statements through ADP Workforce (workforcenow.adp.com) for up to three years.
401(k) changes
To update 401(k) deductions for your final paycheck, submit changes through the Fidelity website at least seven calendar days before the pay date.

After your employment ends, you and your eligible dependents may elect to continue health care coverage through COBRA. To avoid a lapse in coverage, complete your COBRA election within the required election period. You may enroll either by mail (using the enrollment packet sent to your home) or online.
Enrollment options
By mail
A COBRA enrollment packet will be mailed to your home within 14 days after your loss of coverage. Follow the instructions in the packet to enroll.
Online
You do not need to wait for the mailed packet to enroll. Visit CohesityUSBenefits.com to:

401(k) distributions
If you participated in the plan, Fidelity will mail you information outlining your distribution options.
To request a termination distribution, visit www.netbenefits.com. Online requests are generally processed more quickly.
For security reasons, ACH and wire transfers are not available for distributions. Approved requests will be issued by check and mailed via USPS.
Be sure your mailing address is up to date with Fidelity to avoid delays. You can update your address by contacting Fidelity Participant Services at 1-800-835-5097.
Outstanding 401(k) loans
If you have a loan through your 401(k) plan, the full outstanding principal and accrued interest become due after your employment ends. Contact Fidelity promptly to confirm your repayment options and outstanding balance. If the loan is not repaid, it will be treated as a taxable distribution.
